Why use this you ask? Throughout the season, you will be required to fill out a number of waiver forms for each individual tournament. These forms take time to fill out, and can be tedious. By filling out the player information, we can use that data to fill out as much the waiver form as possible. All you have to do is fill in the blanks, print out the form, sign and you are done. No more filling out those long waiver forms! The more information you provide, the less you will need to fill in when completing the waiver forms. Click here to register
